Care Coordinator/Life Skills Instructor - Hamilton Heights Schools

Do you have an interest in providing specialty behavioral health services to consumers and their families in a school, home and community setting? If so, we may have the perfect role for you as a Care Coordinator/Life Skills Instructor on our School Based Team.

Position Summary
This position provides behavioral health and/or child welfare related services to youth and their families in school, community and/or home settings in order to achieve their identified goals.
 
Education
  • Bachelor's degree in social work, psychology, counseling or a directly-related human service from a college/university accredited by the US Department of Education
  • Unrelated bachelor's degree from a college/university accredited by the US Department of Education will be accepted in combination with a minimum of five (5) years experience working directly with families in the child welfare system and other pertinent related experience
Experience
  • Directly related equivalent experience/competencies in past professional and nonprofessional roles
  • Knowledge and application of utilizing social service systems (school systems, special education, child welfare, and/or juvenile probation systems) highly preferred
  • Experience with ages 3 - 18 in a school setting strongly preferred
