Care Coordinator/Life Skills Instructor - SGL

Position Summary

This part-time Care Coordinator/Life Skills Instructor (CC/LSI) position is for our 24-hour Questend Group Home located in Indianapolis, IN. The CC/LSI will provide care coordination, skills training and development to seriously mentally ill adults. Duties include, but are not limited to: orienting clients to psychiatric rehabilitation process, assessing the needs and strengths of clients, teaching problem solving skills, organizing and supervising community based activities for clients, coordinating with clients to develop a person-centered treatment plan for services, developing resources/connections/contacts in the community, assisting clients in obtaining entitlements, and referring/linking clients to needed services.

Education and Experience                                                                                                                                            

  • Bachelor's degree in Social Work, Psychology, Counseling, or related field from a college/university accredited by the US Department of Education preferred
  • If unrelated degree, must have a minimum of one (1) year experience in a service capacity to individuals in need (e.g., children, youth, elderly, physically, emotionally, and/or mentally disabled, special needs, nursing care, etc.) and some related course work as determined through review of college transcripts
  • Work experience in a health care setting with adults who have a serious mental illness preferred


  • Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
  • Must have the ability to use personal car to transport self and others
