The Community Health Worker (CHW) is a trusted member of the community who serves as a liaison/link/ intermediary between health/social services and the community to facilitate access to services. The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support and advocacy.
High School Diploma or Equivalent required; Associates degree preferred
- Achieve and maintain CHW certification or be willing to achieve certification
- Two (2) years experience working with a varity of populations highly preferred
- Knowledge of community, social service systems, community resources, legal systems, and child welfare systems highly preferred
- Must have intermediate computer skills
